“Filler-Up” with Non-Surgical Wrinkle Erasures

Unwanted hair, acne, wrinkles, sun damage, skin laxity - now there is a way to eradicate all of these beauty busters without bandages, scalpels, or time away from work. If this plethora of new technology has you confused, Dr. David Goldberg is here to give you the lowdown.

Hylaform® is THE “optimal solution for lines, wrinkles, lip enhancement, and facial contouring, said world renowned Cosmetic Dermatologist Dr. David J. Goldberg. Hylaform itself is made from hyaluronic acid, a natural substance already found in the skin and can last for up to eight months. Hylaform treatments take less than half an hour and may require only the affected area to be slightly anesthetized. When Hylaform is injected into an area on the face, it naturally replaces the hyaluronic acid we all have in our skin which is lost with time. Results are immediate; and redness, bruising and swelling which are often seen with all existing filler options appear to occur less with Hylaform. Also, Hylaform does not require the skin testing associated with collagen injections.

Hylaform gel works to “fill” and smooth out deep facial furrows, acne scars, and unwanted wrinkles. It can also be used to enhance the border of the lips for those who desire a fuller look. However, for particularly deep smile lines or wrinkles, a second treatment may be required two to four weeks following the initial treatment. Hylaform is ideal when used in conjunction with other lunchtime procedures such as Botox®, Thermage skin tightening, and Photofacials.

The introduction of Hylaform as the most recent method in the continuing battle to reverse the signs of aging comes after a string of popularly used anti-wrinkle injectables such as ArtecollÒ, Botoxâ, Restylaneâ, and RadianceÔ to name a few.

The Foot Facelift: Cosmetic Surgery for your Feet

This little piggy went to market, this little piggy stayed home….Remember that favorite childhood saying? Well, if any of your little piggies hurt or aren’t as attractive as they used to be, why not consider a Foot Facelift? What is a Foot Facelift you say? It is used to describe cosmetic surgery of the feet.

The typical “high heel foot is one which is triangular-shaped in the front like a high heel shoe. The big toe is pointing towards the little toes (bunion) and the pinky toe is pointed towards the big toe. The toes in between are often curled up (hammertoes) instead of straight, and any or all of the toes may have corns on them. With the added pressure on the ball of the foot caused by heels, there are often numerous callouses present on the soles as well.

Many people have a very long second toe. Perhaps you have unsightly corns which keep you from wearing sandals. Maybe your pinky toe kills you in heels or even in your flats. Well, the good news is that toes can be shortened, corns can be cut out, and fifth toes made slimmer and trimmer to fit more comfortably into shoes. Think of it as a “toe tuck for pudgy toes.

Over time, your feet can become so painful that you can’t wear shoes without pain, let alone heels. If that is the case, then you are overdue for the foot doctor. I see many women with “high heel feet in my office, and most of them have moderate to severe pain along with the ugly corns and crooked toes. Most of them require foot surgery even if they’ve switched to “sensible shoes. Today, 99 percent of the time it is an outpatient procedure with the patient able to walk the same day of the surgery and home in just a few hours. Additionally, incisions can be placed so they are inconspicuous, and in some cases out of visual site so that no one will even know you ever had surgery. Recovery times are shorter than ever - sneakers in a few weeks, shoes shortly after that. Full recovery takes about two months, but most patients miss little time at work.

It is gratifying to be able to help women feel better about themselves and at the same time help patients who are suffering with foot pain to get them to walk without fear of pain again. And in many cases that means they can walk in heels again.

Get a picture perfect smile in a SNAP, literally!: New Snap-On Teeth Prove to be an Instant Smile Fixer

Money can buy just about anything these days, including a movie star smile. But that mouthful of pearly white caps and veneers loses some of its shine if it costs tens of thousands of dollars. Now there’s an aesthetic and cost-effective alternative, Snap-On Smile! Once snapped into place, the device can correct the look of tooth flaws to straighten, fill-in, and whiten a smile. Snap-On Smile can even be created to emulate Hollywood’s million-dollar A-listers.

Snap-On Teeth

For those who desire whiter or straighter teeth, this invention is a much quicker and infinitely more cost effective option. I simply make a dental impression of existing teeth in order to create a customized resin prosthetic model which actually snaps onto the back teeth. The patients’ posterior teeth are used as an anchor for the Snap-On Smile. No damage or irreversible treatment is done to the existing teeth. The estimated cost for Snap-On Smile ranges from $1,275 to $1,800. Snap-On Smile is highly recommended for people who have worn, discolored teeth, and for those who have ground down their teeth. They may also be used by patients who are missing several teeth as long as there are posterior teeth present to support the snap-on teeth. Most people opt for the device to enhance their natural smile or to mimic that of their favorite actress/actor.

Before Snap-On Teeth

After Snap-On Teeth

Snap-on Smile serves both functional and aesthetic purposes and can also be used in lieu of dentures. Like dentures, the appliance is removable. However, Snap-on Smile is more accurate and less bulky. Unlike dentures, patients can transform their entire smile while still maintaining their existing teeth because the snap-on teeth are simply placed over the originals. Dentures only replace the missing teeth, and the appearance of the remaining teeth remains unchanged. Additionally, Snap-On Smile can preserve original teeth which have been ground down by TMJ because they serve as a guard to protect the remaining teeth from further occlusal wear. Snap-On Smile can be worn all the time, even while sleeping, and only require removal in order to clean the underlying teeth and device.

Amidst all of the hype: What is breast augmentation?

Women who want larger and shapelier breasts, or who have one breast noticeably smaller than the other, often discover a more positive self-image after a breast augmentation. The intended result is a soft, natural-appearing breast, an improved self-image, and a whole new way to shop for clothes with tops and bottoms that match.

Breast augmentation is performed on an outpatient basis under general anesthesia. A small incision made either in the crease under the breast or in the armpit, and a saline or silicone implant is inserted either below or above the chest muscle, depending upon the need for lift. A soft elastic bra is provided to lend support during the recovery period.

before breast augmentation

after breast augmentation

Initially there are restrictions on activity during recovery time, especially in the use of the arms. Most patients can return to work in two weeks or sooner, and by the end of the month all regular physical activities can be resumed. Sutures are usually removed in about twelve days and massage is begun at the same time.

There is no known association of the implants with any disease. There is no effect on subsequent risk of breast cancer development or in future ability to breast-feed. The implant consists of a fully cured silicone bag filled with intravenous quality saline solution, which is added through a sealed system for extra safety.

What happens if a silicone implants leaks?
The silicone implant is very stabile. Any leakage or rupture can easily be detected via an MRI (Magnetic Resonance Imaging). Most often the only side effects noted are a slight burning, some breast hardness or an irregularity of the breast implant itself. The silicone does not travel through the body, but rather stays confined to a fairly specific area right around the implant (within the capsule) and is quite easily removed and replaced with a new implant.

Can fat be injected into the breast for fullness?
Technically, this is possible and is being done. Fat injection should be avoided, however, because after a few years the fat, much of which dissolves, leaves scar tissue, which cannot be distinguished from cancer. I consider this procedure malpractice.

Does fungus grow in saline implants?
Implant manufacturers now recommend a closed system of filling the implants, which virtually eliminates the risk of fungus getting into the implants.

The Plastic Surgery Workout: True Body Sculpting Developed and Taught by a Plastic Surgeon and Champion Body Builder

Bruce J. Nadler, M.D. training with a patient

Do you find yourself working to the max, but not seeing maximum results in your physique? Based on my years as a plastic surgeon and champion body builder, I’ve developed a regimen which will help you achieve the sculpted look you’ve been striving for.

The Plastic Surgery Workout is different from other workouts in the following ways:

  • Appearance specific
  • Concentrates on how you look, not on sports you play
  • Based on plastic surgery principles of change of shape
  • Begins with careful analysis, comparing present contours with desired contours
  • Exercises muscles unequally, taking into account each individual’s genetic ability to develop those specific areas
  • Decides what needs to be developed and what needs to be reduced
  • Other workout schemes will build muscles or tone the body
  • The plastic surgery workout, based on the analysis, will build some areas, tone others and de-train areas to become smaller
  • Time efficient since only targeted muscles need to be exercised
  • Ongoing analysis to modify the workout, depending on the degree of response in the exercised areas

Analysis is based on the medical SOAP method of organization.

  • Subjective - What is the final look we wish to achieve?
  • Objective - Photographs, exact measurements of body fat, Basal metabolism, limb and torso girth, hormonal blood test values
  • Analysis - How do these measurements differ from the ideal? What muscle must be built up? What has to be maintained by different methods of exercise? What has to be allowed to shrink? What is the correct percentage of body fat?
  • Plan - The exact exercise and diet scheme combining resistance traingin with aerobic exercise

To summarize his program, I use the vowel system

A - appreciation of body form
E - evaluation of changes needed to achieve this form
I - innovative approach to exercise
O - observe the change and constant adjust the routine
U - utilize different modalities of exercise for specific results

Plastic Surgery Diet

  • Not a quick fix
  • Evolving nutritional plan, not a fad diet
  • Based on surgical principles of recuperative nutrition
  • Quick weight loss diets are difficult to maintain.
  • Eating habits must be changed and evolve in stages to be maintained.
  • Too difficult to change all the parameters at once.

Step 1 - Decrease to eliminate refined starchy carbohydrates. Increase protein
Step 2 - Divide food into 5-6 meals a day
Step 3 - Determine portion size and allocation of protein, carbohydrates, and fat
